APPENDIX II

MINIMUM DATA REQUIREMENTS

FOR THE PROOF OF ORIGIN

(Issued together with Circular No. 44/2025/TT-BCT dated 07 July 2025

of the Minister of Industry and Trade providing rules of origin under

the Agreement Establishing the ASEAN - Australia - New Zealand Free Trade Area)

I. Data requirements for the Certificate of Origin

1. Exporter details:

Name, address and specific contact details of the exporter.

2. Consignment details (each Certificate of Origin covers a single importation of goods only):

a) Name and address of the consignee;

b) Information sufficient to identify the consignment, such as the importer's purchase order number, invoice number and invoice date, and the air waybill or bill of lading or transport document;

c) Port of discharge (if known).

3. Full description of the goods:

a) Full description of the goods, including the HS code (at the 6-digit level), the product number and the product brand name (if any);

b) The relevant origin criterion;

c) The FOB value where RVC applies. The FOB value required on a back-to-back Certificate of Origin is the FOB value of the goods exported from the intermediate Member State. For Australia and New Zealand, where a Certificate of Origin or a back-to-back Certificate of Origin does not state the FOB value, that certificate shall be accompanied by an exporter's declaration stating the FOB value of each product described in the certificate.

4. Certification by the issuing authority or body:

On the basis of the information provided, the issuing authority or body certifies that the goods declared in the Certificate of Origin satisfy the rules of origin provided in this Circular.

5. Reference number of the Certificate of Origin:

Each Certificate of Origin is given a reference number by the issuing authority or body.

II. Minimum data requirements for the self-certification of origin document

1. Name and address of the exporter;

2. Name and address of the producer (if known);

3. Name and address of the importer or of the consignee;

4. Description of the goods and the HS code (at the 6-digit level);

5. Where the exporter qualifies, the code or identification number of the exporter or producer;

6. Reference number;

7. Origin criterion of the goods;

8. Certification by the authorised signatory that the goods satisfy the requirements of this Circular;

9. Country of origin;

10. FOB value where the regional value content criterion applies;

11. Quantity of the goods;

12. Where a back-to-back self-certification of origin document is issued:

- State the reference number, date of issue and country of origin of the original proof of origin of the original exporting Member State;

- State the code of the approved exporter of the original exporting Member State (if any).
